To take part, simply make a donation of £2 with "Quiz POST" or "Quiz EMAIL" as your message, depending on whether you would like your quiz sheet posted or emailed - please make sure that you tick the box at the end to say that we can contact you - otherwise we won't be able to send you your quiz! - and make sure you include your email address if you want to complete your quiz online, or print it off yourself!

Comleted quiz sheets can be returned to supporters@ageconcernhampshire.org.uk, or posted to Age Concern Hampshire, Freepost Licence SCE6958, 1 St Cross Road, Winchester, Hampshire, SO23 9JA (if we post you your quiz, we will also send you an envelope) The winner will be selected on WEDNESDAY 17TH DECEMBER AT 3PM so we MUST receive your entry by 12PM on this day!
